A non-refundable registration fee of $200 (made out to Friends United Meeting with the work team listed in the memo section of your check) is to be included with this form. (Registration fee is not required for the Friends United Meeting and Quaker Hill Conference Center Work Team, October 2008.)The total cost of the trip represents all costs except spending money from the point of group departure. Participants age 14 and under must be accompanied by a parent. Participants between the ages of 15-17 must arrange for one of the adult members of the team to be their sponsor during the trip.
Please fill out the form below and mail it to Terri Johns at Friends United Meeting (see address below) with your deposit. Please return a copy of your passport page with your registration and deposit.
